Driver license/ picture ID
Insurance cards
For uninsured patients wishing to apply for the sliding fee discount program please bring (2 current pay stubs, W2 or SSI, etc.)
For Newborns, please remember to bring discharge paperwork

Sliding Fee Scale

Cost should not be a barrier to your healthcare. That's why we provide discounts to families whose household income is at or below 200% of the federal government’s poverty guidelines

To qualify you'll need to bring proof of income for each member of your household (2 current pay stubs, W2, SSI, etc)
