Pediatrics
Please complete the enrollment form and release of information completely and a patient access coordinator will be in contact with you shortly.
Bring the following with you:
- Driver license/ picture ID
- Insurance cards
- For uninsured patients wishing to apply for the sliding fee discount program please bring (2 current pay stubs, W2 or SSI, etc.)
- For Newborns, please remember to bring discharge paperwork

Sliding Fee Scale
Cost should not be a barrier to your healthcare. That's why we provide discounts to families whose household income is at or below 200% of the federal government’s poverty guidelines
To qualify you'll need to bring proof of income for each member of your household (2 current pay stubs, W2, SSI, etc)
