• Acute Care  Evaluation and treatment for acute medical concerns such as urinary tract infections, upper respiratory infections, and skin infections.

• Chronic Disease Management  Diagnosis and treatment of chronic health problems such as diabetes, high blood pressure, high cholesterol, arthritis through an integrated care plan of checkups, screenings, and monitoring.  We do not prescribe long term controlled medications (opiates, benzodiazepines, stimulants).

• Immunizations Vaccinations to protect you from a variety of diseases including seasonal flu shots

• Cancer Screenings or referrals for breast, cervical, colon, and prostate cancers

• Case Management We can help you overcome obstacles to accessing care
